Take Control of Your Health Journey with ELEVATE
Dr. Jacob Jacobs built Elevate Physical Therapy and Training to be more than just a rehab clinic: it is a place where clients can regain strength, prevent future injuries, and take control of their long-term health. With a background in athletics and advanced training as a Doctor of Physical Therapy (DPT) and Certified Strength and Conditioning Specialist (CSCS), he understands both the science of movement and the challenges that come with staying active. His goal is to help clients not only recover but build resilience, improve mobility, and feel their best for years to come.
Dr. Jacobs’ journey into physical therapy is deeply personal. Growing up as an athlete, he knew firsthand the challenges of staying active and healthy while dealing with injuries. His experiences on the field and in the gym gave him a unique understanding of the struggles that come with recovering from physical setbacks. More than just a physical injury, being sidelined can take a mental and emotional toll on an athlete. For Dr. Jacobs, this ignited a deep curiosity about how the body heals and how individuals can bounce back stronger.
“It’s about more than just treating pain. We want to empower our clients to take control of their health. Our goal is for people to understand how their body moves, how to strengthen it, and how to make smart choices that support their long-term well-being.”
– Dr. Jacobs
“I’ve always been fascinated by the way the body works,” Dr. Jacobs says. “When I experienced my own injuries, I wanted to learn everything I could to not only get better but to prevent future setbacks. It became a passion for me, and I knew I wanted to help others in the same way.”
This passion led him to pursue a Doctorate degree in Physical Therapy. After completing his education, Dr. Jacobs quickly realized that traditional physical therapy could sometimes be limited in scope. While recovery is essential, he wanted to help his clients go beyond just getting back to baseline. They deserved to thrive and achieve new physical heights. This realization set the foundation for his approach to physical therapy, which is both rehabilitative and performance oriented.
Dr. Jacobs always knew he wanted to open his own practice, but the opportunity came sooner than expected. During his final clinical rotation at BioFit – Therapy Based Fitness, he saw firsthand how their personalized, science-based approach to fitness aligned with his own vision for helping people move pain-free and build long-term strength. Upon graduating, BioFit offered him the opportunity to be an independent contractor out of their gym, providing a supportive environment where expert trainers and healthcare professionals work together to help clients achieve their best results.
As the owner of Elevate Physical Therapy and Training, Dr. Jacobs has built a practice that focuses not only on rehabilitation but also on injury prevention, performance enhancement, and overall wellness. Dr. Jacobs believes that physical wellness is the key to a fulfilling life, no matter what one’s age or stage of life. He is dedicated to helping his clients achieve long-term health, whether they are elite athletes or people looking to stay active as they age.
At Elevate, clients receive a holistic approach to their health. The treatment plan is customized to the individual’s unique needs, goals, and lifestyle. Dr. Jacobs and his team do not simply treat symptoms; they address the root cause of the problem, ensuring that the client has the tools and knowledge to prevent injuries in the future. This thorough, personalized care is what sets Elevate apart from other physical therapy clinics.
Dr. Jacobs explains, “It’s about more than just treating pain. We want to empower our clients to take control of their health. Our goal is for people to understand how their body moves, how to strengthen it, and how to make smart choices that support their long-term well-being.”
One of the things that Dr. Jacobs values most in his practice is the personal connection he forms with his clients. He takes the time to truly understand each person’s journey, challenges, and goals, ensuring that their path to recovery is both effective and individualized.
“I believe that the relationship between a patient and their physical therapist is fundamental to their success,” Dr. Jacobs shares. “When clients feel heard and understood, they are more likely to engage in their recovery process. It’s not just about exercises and stretches, it’s about helping them feel supported and confident in their journey.”
Accessibility is a key part of Dr. Jacobs’ vision for Elevate. He is proud to offer online physical therapy services, which extend the reach of Elevate beyond the walls of the clinic. His online platform allows him to work with clients regardless of their location, whether they live in a different city, are traveling for work, or simply prefer the convenience of virtual sessions.
“Online physical therapy is a game-changer,” Dr. Jacobs says. “We can help people who might not have access to quality care in their area, or who need more flexibility with their schedules. It’s not just about the technology – it’s about making high-quality care available to anyone who needs it, no matter where they are.”

Dry Needling is a practice Dr. Jacobs offers to help relieve tight muscles and joints.
This new approach offers clients the flexibility to continue their physical therapy sessions while traveling or working remotely. For those who may find in-person sessions cost-prohibitive, the online platform provides an affordable option without sacrificing the quality of care. Dr. Jacobs believes that this approach opens up physical therapy to a much wider audience, allowing people to access the treatment when they need it most.
Elevate’s online services feature virtual sessions, where Dr. Jacobs personally guides clients through exercises, monitors their progress, and makes real-time adjustments to their treatment plans. This approach ensures that clients can remain actively involved in their recovery, regardless of their location.
Elevate also offers a range of online programs designed to complement virtual sessions or serve as standalone resources for those who prefer a structured plan. In addition to personalized virtual consultations, clients can access targeted exercise programs focused on specific areas like knee, shoulder, or lower back pain. These programs provide guided exercises, video demonstrations, and ongoing updates to support long-term progress, giving clients the flexibility to work on their recovery at their own pace.
Dr. Jacobs’ ultimate mission with Elevate Physical Therapy and Training is to guide clients toward a path of lifelong wellness. He believes that taking care of your body is a lifelong commitment, and he is dedicated to helping his clients build sustainable habits that support their physical well-being. Through a combination of expert guidance, personalized care, and innovative tools, Dr. Jacobs helps people not only recover from injury but also enhance their performance, build resilience, and improve their quality of life.
“In the end, it’s about living well and staying healthy for years to come,” he says. “Physical therapy is just one tool in that process. It’s about building a lifestyle that supports long-term vitality, and I’m honored to be a part of that journey for so many of my clients.”
Dr. Jacobs is excited about the future of Elevate Physical Therapy and Training. With the success of his online platform, he plans to continue expanding and improving his virtual services to reach even more clients. As the world becomes increasingly digital, he sees online physical therapy as an important part of the future of healthcare, allowing individuals to take charge of their own wellness and access expert care on their terms.
“I am committed to constantly learning and improving to ensure that our clients receive the best care possible,” Dr. Jacobs says. “Whether in person or online, our goal is always the same: to help people live better, healthier lives.”
